This form lets Next 2 Play capture and publish images of your player in sessions, social media, and promotional materials. You can withdraw consent at any time.

We generate a PDF in Spanish or English. Print it or sign it digitally.

Only a parent or legal guardian can sign. Make sure to include the player's name and the date.
